the following information is designed to answer many of the questions that most patients have.
practice hours:
monday - thursday
8:00am - 1:00pm
2:00pm - 5:00 pm (lunch at 1pm)
appointments:
we see all patients on an appointment basis and we request that you call in advance so that we can reserve ample time for you. we make every effort to honor all time commitments. because we are very busy, we ask that you notify us 48 hours in advance if you need to reschedule your appointment.
broken appointments:
failure to give 24 hours notice upon the second broken appointment will result in the inactivation of your chart. special circumstances will be taken into consideration.
emergency care:
we realize that emergencies happen and we will do our best to respond to an emergency situation.
financial procedures:
we make every effort to keep the cost of your dental care down and request that you help by paying upon completion of each visit.
insurance:
we do offer the courtesy of submitting your claims for you, but require that you pay your deductible and estimated percentage of the service performed at the completion of each visit. if you wish to have your insurance submitted by our office, we will need a current copy of your dental insurance card. we will do our part to help maximize your insurance benefits.
we cannot submit insurance for children of non-custodial parent.
please remember that insurance is considered a method of reimbursing the patient for fees paid to the doctor and is not a substitute for payment. student part time job online without investment after sixty (60 ) days, all unpaid balances (regardless of insurance coverage) are the responsibility of the patient.
